  • 1-25 Licensed Beds: $750 annually
  • 26‐100 Licensed Beds: $1,500 annually
  • 101–300 Licensed Beds: $2,500 annually
  • 301–600 Licensed Beds: $5,000 annually
  • >600 Licensed Beds: $7,000 annually
